Use of Lymphatic Wire for Lymphatic Surgery

はじめに

　リンパ浮腫に対する外科治療において，リンパ管の同定方法はさまざま工夫が蓄積されてきたが，吻合に適したリンパ管の最適解はまだ得られていない。リンパ管ワイヤーはインドシアニングリーン（以下，ICG）蛍光リンパ管造影や超音波検査では描出できない，皮膚表面から離れたリンパ管吻合部位探索の方法の1つとして提案された手法であり 1），特別な設備なく使用可能であるが，手技にはいくつかのポイントが存在する。リンパ管ワイヤーの使用による臨床的な有用性，中・長期的な治療成果については今後の報告を待つ必要があるが，本稿においては，臨床使用における手技上のポイントについて解説する。

　In the surgical treatment of lymphedema, various devices have been used for the identification of lymphatic vessels, but the optimal method for the identification of lymphatic vessels that are suitable for anastomosis has not been established. Lymphatic wire was proposed as a material that can be used to search for the best site for a lymphatic anastomosis and is already available for clinical use, but there are several points to consider when using it. It is necessary to wait for future reports on the clinical benefits, medium- and long-term results, and safety of lymphatic wire use. In this article, we describe the key points of the lymphatic wire technique in clinical use.
