Charcot関節を股関節に生じた後腹膜平滑筋肉腫の1例

A case of Charcot’s arthropathy occurring the hip joint due to retroperitoneal leiomyosarcoma

Charcot関節は神経障害性関節症とも呼ばれ，種々の中枢・末梢神経疾患に合併する。神経感覚障害に続発する骨と関節の破壊性疾患で，関節の進行性の変性を生じ，最終的に関節構造の強い破壊が生じ，脱臼と関節周囲の骨片を特徴とする^1）。主に糖尿病性ニューロパチーに合併し足や足関節に好発するが，Charcot関節が股関節に生じた報告は少ない。今回我々は，後腹膜の平滑筋肉腫に起因した股関節のCharcot関節の1例を経験したので，文献的考察を加えて報告する。

We report a rare case of Charcot’s arthropathy of the right hip joint due to retroperitoneal leiomyosarcoma. A man in his 70s presented with lower limb weakness and bowel dysfunction, which was diagnosed as neurological impairment due to vertebral invasion by the tumor. Additionally, progressive destruction of the right hip joint had been noted several months prior to admission. This was attributed to the development of Charcot’s arthropathy resulting from sensory impairment of the hip caused by the retroperitoneal tumor.
