下腹壁動脈の医原性損傷に対する動脈塞栓術の検討

Evaluation of transcatheter arterial embolization for iatrogenic inferior epigastric artery bleeding

下腹壁動脈（inferior epigastric artery：IEA）は腹壁を栄養する主要血管の1つだが，同IEAからの出血は時に重篤なショック状態を引き起こしうる^1-3）。一方，出血に対する経カテーテル的動脈塞栓術（transcatheter arterial embolization：TAE）は一般に低侵襲かつ有効であるが，特にIEA出血の約3割^2）を占める医原性損傷に対するTAEについてはまとまった報告はみられない。今回我々は，医原性IEA出血に対するTAEについてretrospectiveに検討したので報告する。

We retrospectively evaluated outcomes of transcatheter arterial embolization（TAE）for iatrogenic inferior epigastric artery（IEA）bleeding in eight patients caused by laparoscopic port placement or paracentesis et al. The technical and clinical success rates were 100％, 87.5％, respectively. One procedure-related complication（12.5％）occurred. The 30-day mortality rate was 25％（2 patients）, influenced by factors such as underlying progressive malignancy. TAE was effective for iatrogenic IEA bleeding, but patient background and potential complications warranted careful consideration.
