anomalous unilateral single pulmonary vein（AUSPV）は片側の1つまたは2つの肺静脈が欠損し，蛇行・拡張した還流静脈が同側の肺静脈に合流する肺静脈疾患である。過去に30例程度しか報告されていない非常にまれな疾患であり，治療例や死亡例の報告はなく，基本的には治療の対象とはならない。今回我々は，偶発的に発見されたAUSPVの1例を経験したので，文献的考察を加えて報告する。

Anomalous unilateral single pulmonary vein（AUSPV）is a rare pulmonary vein disorder where one or two veins are absent, and collateral veins drain into the ipsilateral pulmonary vein. With about 30 reported cases, it is typically asymptomatic and requires no treatment. This case involved a teenage male diagnosed incidentally via imaging. CT and pulmonary angiography confirmed AUSPV, differentiating it from arteriovenous malformations and pulmonary vein varices. Since AUSPV does not require treatment, accurate diagnosis is critical to avoid unnecessary interventions and to distinguish it from other vascular anomalies.
