肺動静脈奇形を疑われ，肺動脈造影にて診断したmeandering pulmonary veinの1例

Meandering pulmonary vein mimicking pulmonary arteriovenous malformation and diagnosed by pulmonary arteriography：a case report

meandering pulmonary vein（MPV）は1972年にGoodmanらにより報告された，良性の先天性肺血管異常であり^1），これまで20数例の報告しかないまれな肺静脈の変異である。MPVはしばしばscimitar症候群や肺動静脈奇形（arteriovenous malformation：AVM）と誤認されることがあるが，MPVの静脈灌流は最終的に左心房へつながる同所性であり，シャントを生じず，単独のMPVでは治療の必要はない^2）3）。今回，肺AVMが疑われ，肺動脈造影を行った結果，MPVと診断した1例を経験した。

Meandering pulmonary vein（MPV）is a rare congenital pulmonary vascular anomaly, which is confused with scimitar syndrome and pulmonary arteriovenous malformation（AVM）. We report a case of MPV which mimics pulmonary AVM. A woman in her 60s was referred to our hospital for an abnormal vessel in right lung detected accidentally during treatment for pleurisy. Initially, pulmonary AVM was suspected. Pulmonary angiography showed normal pulmonary artery but abnormal vessel that drain to right inferior pulmonary vein. Therefore, the patient was diagnosed as MPV.
