画像で門脈が腫瘤を貫通している所見がみられたときには，悪性リンパ腫や胆管細胞癌がまず疑われ，肝細胞癌でみられることは極めてまれである。今回，本所見を呈した高分化肝細胞癌の2例を経験したので，若干の文献的考察を加えて報告する。

We reported two cases of hepatocellular carcinoma with an interesting finding that the portal vein penetrated within the mass. In case 1, the portal vein penetrated a well-differentiated hepatocellular carcinoma of about 45mm in size of the simple nodular type with expansive growth. No residual portal area was found within the mass. In case 2, the portal vein penetrated a dysplastic nodule of about 30mm in size bordering a well-differentiated hepatocellular carcinoma of about 10mm in size. The dysplastic nodule had residual portal area. We speculate that the portal vein may rarely penetrate within a well-differentiated hepatocellular carcinoma that is dedifferentiated while the stromal invasion is poor.
