異所性肝は主肝との結合をもたない肝臓組織であり，発生頻度は低く，その中でも胆嚢に生じやすいとされている。また，通常の肝細胞癌は，肝炎ウイルス感染例など高い危険群での発生が多いとされるが，異所性肝の場合はそれらの背景因子がない場合でも肝細胞癌の発生率が比較的高いとの報告がある。今回我々は，骨盤腔内の異所性肝に生じた肝細胞癌の1例を経験したので，文献的考察を加えて報告する。

A 74-year-old man with no history of previous hepatic disease presented lower abdominal pain and fever. Contrast-enhanced CT revealed a tumor in the left pelvic cavity, and hydronephrosis of left kidney. There was no hepatocellular carcinoma（HCC）in primary liver. Operation was performed, and the tumor adhered to mesentery of sigmoid colon and ileum. Also there was peritoneal dissemination, surgeons performed partial resection. Histological diagnosis was low-grade HCC, considered from ectopic liver in pelvic cavity.
