Ⅲ期非小細胞肺癌に対するIMRT：物理・技術的ポイント

Intensity-modulated radiation therapy for stage Ⅲ non-small cell lung cancer：physical and technical points

Ⅲ期非小細胞肺癌に対する根治的化学放射線療法後に維持療法として免疫チェックポイント阻害薬を使用することにより，無増悪生存割合および全生存割合が大幅に改善された^1）2）。本レジメンは同癌に対する従来の化学放射線療法と置き換わり，新しい標準治療となった。一方で，維持療法に使用する本薬剤はgrade 2以上の間質性肺臓炎を有する患者には適用できない。本薬剤の幅広い適用には，化学放射線療法時の放射線肺臓炎を低減するための照射技術が必要であった。その照射技術として注目されたのが強度変調放射線治療（intensity modulated radiation therapy：IMRT）であり，2016年のメタ解析^3）において，IMRT群は3次元原体照射（3-dimensional conformal radiation therapy：3DCRT）群と比較してgrade 2以上の重篤な肺臓炎が少なかったと報告された。当院では，本薬剤の保険承認と時期を同じくして2018年から同癌に対する強度変調回転放射線治療（volumetric modulated arc therapy：VMAT）を実施してきた。本稿では，同癌に対するIMRT・VMATのさらなる普及を期待して，当院における経験をもとに，前半では物理・技術的な視点から固定具および呼吸性移動対策を紹介し，後半では治療計画の線量最適化計算における重要なポイントを解説する。

This paper describes the optimization of patient fixation and respiratory motion management from physical and technical points of view, related to intensity-modulated radiation therapy for stage Ⅲ non-small cell lung cancer. In the latter half, we explain the important points in the dose optimization calculation of treatment planning：relaxation of dose concentration to planning target volume（PTV）for dose reduction in normal lungs, optimization of collimator and gantry angles, dose reduction in the heart and coronary artery, and the split PTV technique.
