肺癌に対するIMRT：コンツーリングのコツとピットフォール

IMRT for lung cancer：contouring tips and pitfalls

肺癌強度変調放射線治療（intensity-modulated radiation therapy：IMRT）放射線治療計画は理論・機器・装置の進歩により大きく変化してきている。その中で放射線腫瘍医には適切な診察と画像診断を基としたターゲットの描出（コンツーリング）が求められる。コンツーリングは ① 肉眼的腫瘍体積（gross tumor volume：GTV）の同定，② CT撮像とdata-set作成，③ ターゲットコンツーリングの3段階を経て行われる。本稿では各段階における日常臨床でのコツ・ピットフォールについて概説する。

Contouring in radiotherapy planning for lung cancer consists of the identification of GTV, CT imaging for treatment planning, creation of CT data-set if necessary, and target contouring. The CT images for treatment planning needs image quality which will be used not only for contouring but also for dose calculation and set-up during treatment. In CTV contouring of lung field lesions, it should be noted that the tumor margins on CT images do not agree with those on pathological specimens. The planners are needed their own policy for target contouring.
