肺原発の粘表皮癌（pulmonary mucoepidermoid carcinoma：PMEC）は気管支腺の導管上皮より発生するまれな悪性腫瘍で，約半数は30歳未満に生じる。粘液産生細胞，扁平上皮細胞あるいは扁平上皮細胞様細胞，および中間細胞の混在から構成される^1-3）。低悪性度と高悪性度の2段階に分類され，大部分は低悪性度の病変である。ブラ壁近傍発生の肺癌は腺癌や扁平上皮癌，大細胞癌の報告が多いが^4），今回，ブラ壁近傍発生の粘表皮癌を経験したので報告する。

Pulmonary mucoepidermoid carcinoma is a rare malignant tumor arising from the mucous glands of the bronchus. The tumors are usually well differentiated and contain a combination of mucus-secreting, squamous, and intermediate cells. They usually occur in the central bronchi（main bronchi, lobar bronchi, and segmental bronchi）, and rarely occur in the trachea or peripheral lung parenchyma. We report an extremely rare case of peripheral type mucoepidermoid carcinoma adjacent to the bullous wall.
