FDG集積を示した縦隔リンパ節を短期間で経過追跡しえたサルコイド反応の1例

A case of sarcoid reaction in lymph nodes with FDG accumulation

FDGは悪性腫瘍だけでなく炎症や良性腫瘍にも集積しうるため，良悪性の確実な鑑別方法にはならず，症状や血液検査，他の画像検査などから総合的に判断する必要がある。また，悪性腫瘍の化学療法中やその後の経過観察中に出現するサルコイド反応は，リンパ節転移や悪性リンパ腫などとの鑑別が困難である。今回我々はFDG-PET/CTで異常集積を認めた縦隔リンパ節腫大が数カ月の経過で硝子化結節へと変化し，1年でリンパ節腫大と異常集積が低下したサルコイド反応の経過を追跡することができたので報告する。

Abnormal and increased uptake in mediastinum was demonstrated on 18 F-fluorodeoxyglucose-positron emission tomography/computed tomography（FDG-PET/CT）in a 57-year-old woman 7.5 years after completion of surgery, radiotherapy, and endocrine therapy for breast cancer. Considering that mediastinoscopic biopsy several months after revealed hyalinized nodules, and enlargement and abnormal uptake for mediastinal lymph nodes resolved on a followed-up FDG-PET/CT, we diagnosed initial abnormal mediastinal uptakes corresponded to a sarcoid reaction. Although sarcoid reactions are sometimes difficult to be differentiated from malignant diseases such as lymph node metastasis and malignant lymphoma, careful and detailed follow-up in a short period can be useful.
