褐色脂肪腫は軟部組織に好発する腫瘍であるが，骨に発生することはまれである。骨原発の褐色脂肪腫は世界的にも報告例が少なく，2019年までに33例報告されている^1）が，邦文での報告は我々が検索しえた範囲では見当たらなかった。今回，経皮的CTガイド下骨腫瘍生検により診断した仙骨に発生した褐色脂肪腫を1例経験したため，文献的考察を加えて報告する。

Intraosseous hibernoma is a rare benign bone tumor, with 33 cases reported. In this report, we present a case of an intraosseous hibernoma in the sacrum of a 60-year-old female. Intraosseous hibernoma showed heterogeneous osteosclerosis on CT and heterogeneous high signal on T2-weighted images with MRI, but lacks specific findings. CT-guided bone biopsy was performed on the tumor and was relatively minimally invasive for diagnosis. We performed CT-guided bone biopsy on the tumor and were able to diagnose intraosseous hibernoma with relatively low invasiveness. Although it is very rare, intraosseous hibernoma should be considered when imaging demonstrates a fat-containing lesion in spine and pelvis.
