History of tau PET ligand development

2020年5月に[¹⁸F]Flortaucipir（Tauvid^TM，[¹⁸F]AV-1451，[¹⁸F]T-807）が世界初のタウPET薬剤として米国食品医薬品局から承認された。現在，タウPET薬剤の開発はアカデミアだけでなく製薬会社も参入しており，臨床評価まで行われているもので10個以上存在する。基本的にこれらはアルツハイマー病（AD）の脳内に蓄積した線維化タウ（paired helical filaments：PHF）を主成分とする神経原線維変化を標的として開発されているが，薬剤によって結合親和性，結合サイト，薬物動態およびオフターゲット結合の種類・程度が異なる。本稿では，タウPET薬剤の開発の歴史を筆者らの開発したTHKシリーズを中心に解説し，第一世代と呼ばれているタウPET薬剤の問題点について整理し，その問題点を克服するべく開発されている第二世代と呼ばれているタウPET薬剤について概説したい。

Recently,[¹⁸F]Flortaucipir（Tauvid^TM,[¹⁸F]AV-1451,[¹⁸F]T807）was approved by Food Drug Administration in US to estimate the density and distribution of aggregated tau neurofibrillary tangles in adult with cognitive impairment who are evaluated for Alzheimer’s disease. Many tau PET tracers have been developed by academia and pharma and tested in humans over the last 10 years. Although tau PET tracers are designed to detect aggregated tau neurofibrillary tangles found in Alzheimer’s disease, they possess different binding affinity, different binding sites, different pharmacokinetics, and different degree of off-target binding. In this chapter, we will review the history of the development of tau PET tracers and the current status of tau PET radiopharmaceuticals.
