膵管内乳頭粘液性腫瘍（intraductal papillary mucinous neoplasm：IPMN）は日常よくみられる疾患である。これまでにIPMNのFDG-PET/CTに関する文献は散見され，有用性を示したものが多い^1-5）。文献によると，膵管内乳頭粘液性腺癌（intraductal papillary mucinous carcinoma：IPMC）では膵管内乳頭粘液性腺腫（intraductal papillary mucinous adenoma：IPMA）に比べてFDG集積の程度が強い，後期相で早期相よりも集積が上昇する，と報告されている。しかし，ある報告のデータを参照すると，high grade dysplasiaやinvasive carcinomaの症例でFDGの集積が弱いという結果もある^6）。そこで我々は，IPMCでも充実部が微小病変の場合はFDG集積が偽陰性になることもあるのではないかと考えた。今回，術前にFDG-PET/CTを施行したIPMNの症例を対象として，IPMCとIPMAでのFDGの集積の程度を，微小病変群を含めて比較検討したので報告する。

We evaluated FDG uptake of IPMN dividing into the group of IPMC and IPMA. For qualitative analysis, we evaluate if there was visible FDG uptake on tumor. For quantitative analysis, we measured SUVmax on the tumor on early and delayed phases. In patients with the nodule of＜1 cm, the numbers of patients with visible FDG uptake on early phase were 1 for IPMC and 0 for IPMA, and there was no significant difference（p＝0.36）. The average SUVmax on the group of IPMC and IPMA was 2.19 and 1.97, respectively. In patients with IPMC, there are patients with false negative if there was no or a small nodule.
