膵生検で組織を得ることはしばしば困難なため，画像上，悪性が疑われる膵腫瘍に対する膵切除が病理組織の確定なしに行われることがある。結果として，良性病変が外科的に切除されることがあり，そのような疾患の一つとして，膵臓の脂肪性偽性肥大（lipomatous pseudohypertrophy：LPH）がある。今回我々は，脂肪肉腫と画像上，鑑別が難しかったLPHの1例を経験したので，文献的考察を加え，報告する。

A man, who was seventy years old, suffering constipation with anemia. He was diagnosed as colon cancer at descending colon by endoscopy. Giant fat containing tumor was detected in retroperitoneal space by CT and MRI. The tumor with unclear border for surround tissue involved pancreatic tail. Soft tissue nodules without FDG accumulation on PET CT were detected in the tumor. Preoperative diagnosis was retroperitoneal liposarcoma with undiffereciated components. Retroperitoneal tumor was removed. The pathology was lipomatous pseudohypertrophy of the pancreas.
