成人腸重積症は悪性リンパ腫などの腫瘍性病変が先進部となり発症することが多く，内翻Meckel憩室が原因となるのは3％程度と非常にまれである^1）。中でも，腫瘍性病変を伴う内翻Meckel憩室による成人腸重積症の報告例は非常に少ない^2）。今回我々は，漿膜下層内に脂肪腫を伴った内翻Meckel憩室による成人腸重積症を経験したため報告する。

We herein report an adult case of intussusception caused by inverted Meckel’s diverticulum with subserosal lipoma. Enhanced computed tomography（CT）was performed and ileo-ileal intussusception with lipoma was suspected. Emergency surgery revealed a 9×4cm mass in the ileum. A pathological assessment confirmed the subserosal lipoma in Meckel’s diverticulum.

Intussusception caused by inverted Meckel’s diverticulum is uncommon and lipoma developing within a Meckel’s diverticulum is extremely rare. Retrospective CT imaging indicated possible Meckel’s diverticulum, but could not confirm the involvement of a lipoma.
