Imaging biomarkers for diagnosing neurodegenerative disease in dementia

認知症における画像診断の役割は古典的には，治療可能な認知症鑑別のためのスクリーニング検査であった。鑑別すべき疾患としては，血管障害の他，脳腫瘍，正常圧水頭症等が例として挙げられる。これらの疾患を鑑別することは現在でも当然ながら画像の重要な役割であるが，加えて，装置の進歩や統計学的画像解析の手法の導入，分子イメージングの実現により，認知症原因疾患の早期診断あるいは発症前診断に関しても画像が重要な役割を果たすようになってきている。近年，神経変性疾患の国際的な診断基準にバイオマーカーとして画像検査が取り入れられることも多い。本稿ではこれら背景と具体的に認知症の原因疾患を早期に診断するための手法について，総合画像診断という観点から述べる。

Classically the role of imaging in diagnosing neurodegenerative disease in dementia, curative disease like normal pressure hydrocephalus, vascular dementia, or tumors. Recently, owing to development of technique, imaging modalities are becoming main biomarkers with international consensus. Though the more modalities are referred, the more accurately diagnosed, but from the economical and clinical point of view, here mainly MRI, single photon examinations, and neurostatistical imaging those are covered with insurance in Japan, are described. From now on, after disease modifying therapies were achieved, molecular imaging must become more stable and absolute modalities in the near future.
