静岡県の産科有床診療所における大地震対策—現状と課題

　本研究の目的は産科有床診療所における大地震対策の現状と課題を明らかにすることである。本研究では、静岡県内の産科有床診療所6施設に勤務する8名の看護管理者の協力を得て、「施設の概要」、「災害への備え」、「発災直後の想定対応」の3項目について構造化インタビュー調査を実施した。結果は内容別にまとめ、現状と課題に関する看護管理者の思い・考えを質的帰納的に分析した。

　産科有床診療所では、大地震による被害を想定し、対象者の安全確保と診療継続に備える努力をしていた。一方、看護管理者は現状に危機感を抱いており、改善のための予算や有用な情報が充分でないことが示された。今後の課題は、同規模の診療所間で有用な情報共有を行い、基幹病院との災害時連携を検討することである。

The purpose of this study was to clarify the current state and issues concerning obstetric clinics' countermeasures against great earthquakes. In this study, I had a structured interview including a “summary of the facilities,” “disaster preparations,” and “measures considered necessary immediately after the occurrence of an earthquake” with the cooperation of 8 nursing managers who work at 6 obstetric clinics in Shizuoka prefecture. The results are summarized by item, and the views and ideas of the nursing managers were then analyzed qualitatively and inductively.

The current state; namely, that the obstetric clinics had imagined the possible damage caused by a great earthquake and had then made efforts to ensure the safety and continuous treatment of users, became clear. On the other hand, it also became clear that those nursing managers had a sense of crisis concerning the current status, situation, budget, and that useful information for improvement was not sufficient. Future issues are to share useful information with multiple clinics, and to consider collaboration with core hospitals at the time of a disaster.
