腸重積を繰り返す巨大大腸脂肪腫に対して行った腹腔鏡補助下大腸切除術の1例

A case of repeated colonic invagination associated with a lipoma treated by laparoscopy-assisted surgery

　大腸脂肪腫は比較的まれな良性腫瘍であり，その多くは小さく，臨床的に問題になることは少ない．しかし，ときに3cm以上のものは繰り返す腹痛や下血を伴う腸重積の原因となる．今回われわれは，上行結腸の径8cmの脂肪腫が原因で繰り返して発症する腸重積に対して有用であった腹腔鏡下手術の1例を経験した．症例は62歳，男性．数年前から繰り返す右下腹部痛と下血で入院した．腹部超音波，腹部CT，大腸内視鏡検査により最大径8cm大の上行結腸脂肪腫による腸重積と診断し，腹腔鏡補助下に回盲部切除術を施行した．病理学的には分化度の高い脂肪腫で，悪性所見は認められなかった．本症例のように腫瘍径が大きく内視鏡的切除が困難な脂肪腫に対しては，低侵襲である腹腔鏡下切除がきわめて有用であると考えられた．

Colorectal lipoma is a comparatively rare benign tumor.The masses are typically small in size and are clini-cally insignificant. However, tumors 3 cm and larger can sometimes cause intussusception with recurrent ab-dominal pain and melena. In this report, we describe a case in which laparoscopic surgery was effective for repeated intussusception of the ascending colon due to a lipoma 8 cm in diameter. A 64-year-old man was ad-mitted to the hospital complaining of right lower abdominal pain over the last few years and occasional melena.
