大腸の脂肪腫，腸間膜嚢腫はそれぞれ比較的まれな良性疾患である．また両者を合併することは非常にまれで，一期的に腹腔鏡下に切除したという報告はわれわれが検索しえた限り自験例が1例目であった．患者は64歳の女性で，2003年11月に両側髄膜腫の診断のもと右側髄膜腫に対し開頭腫瘍摘出術を受けた．術後貧血が続いたため精査目的にて当科に紹介された．大腸内視鏡検査で横行結腸に5cm大の粘膜下腫瘍を認め，腹部CTで上行結腸外側に径4cm大の嚢胞性腫瘤が描出されたため，2004年1月，腹腔鏡下に切除術を施行した．上行結腸外側より腸間膜嚢腫を切除後，横行結腸を授動後4cmの小開腹下に結腸を切開し，脂肪腫を摘出した，術後経過は良好で第10病日軽快，退院となった．異なる部位に局在する2病変に対し腹腔鏡下手術は有用であった．
A 63 year-old woman, who was in post-operative state of right cerebral meningioma, was introduced to us due to persistent anemia. Colonoscopy revealed a submucosal tumor measuring 5×4 cm in the transverse colon. Computed tomography revealed a cystic mass at the right side of the ascending colon. In order to explore the cystic lesion and resect the tumor, we performed laparoscopically assisted surgery. The mass was located in the right paracolic gutter, which proved to be a mesenteric cyst. First, this cystic mass was resected, and the trans-verse colon was dissected from hepatic and splenic flexure.
