術前に診断しえた閉鎖孔ヘルニアに対し，腹腔鏡下にプラグ法を行った1例を経験したので報告する．症例は85歳の女性．繰り返し出現する，右大腿内側に放散する右下腹部痛のため来院．骨盤CT検査にて右恥骨筋と外閉鎖筋の間に腸管が認められ，右閉鎖孔ヘルニアと診断し手術を施行した．腹腔鏡による経腹到達法で閉鎖孔に回腸の一部が嵌頓していることを確認した．小腸嵌頓を解除し，ヘルニア嚢を内翻して切開し，閉鎖孔を露出した．閉鎖孔にはマーレックス®メッシュプラグを挿入固定し，腹膜を閉鎖した．閉鎖孔ヘルニアに対する腹腔鏡下プラグ法はtension freeであり，再発の可能性が低く，低侵襲で安全かつ容易に行うことが可能な優れた術式であると考える．
We report a case of laparoscopic obturator hernia repair using Merlex® mesh plug, who was diagnosedpreoperatively. The patient, a-85-year-old woman, was admitted to our hospital because of severe rightlower abdominal pain and numbness extending down the inner side of the right thigh to the knee. Pelvic CTshowed incarcerated intestine between the right pectineal muscle and external obturator muscle. Thus, shewas diagnosed as having obturator hernia and laparoscopic operation was performed. The hernia wascarefully reduced and a peritoneal flap was developed.
