◆要旨：患者は39歳，女性．夕食後の右季肋部痛を主訴に紹介されCT検査，MRCP検査で胆石発作と診断された．既往歴として2年前，臍頭側の白線ヘルニアに対して腹腔鏡下メッシュ修復術が行われていた．メッシュ損傷なしに腹腔鏡下手術を行う目的に3DCTでメッシュの存在範囲を確認し，メッシュ留置部を避けて右腹直筋外側に小開腹創を置き単孔式腹腔鏡下胆囊摘出術を行った．術後3日目に合併症なく退院した．上腹部にメッシュが留置されている症例において，3DCTを用いてメッシュの存在範囲を確認することは，メッシュ損傷なしに腹腔鏡下胆囊摘出術を行う際のポート位置決定のために有用な方法であると考えられた．
A 39-year-old woman was admitted to our hospital for right hypochondrial pain. Abdominal computed tomography （CT） and magnetic resonance imaging （MRI） showed gallbladder stone. She had previously undergone laparoscopic mesh repair of linea alba hernia. After confirming the range of the mesh using 3DCT, the patient underwent a single incision laparoscopic cholecystectomy, avoiding mesh damage. There were no perioperative complications, and the patient was discharged on postoperative day 3. In order to avoid the mesh damage in a patient with previous mesh repair for ventral hernia, 3DCT is useful in determining port position.
