◆要旨：TAPP法におけるメッシュ展開の手技は施設によって様々である．パリテックス ラップ プログリップTMメッシュはマイクログリップが組織に固定するため，タッキングが必要ないが，腹腔内でメッシュが大網や腸管・腹膜などの意図しない組織に接着する欠点が存在する．筆者らは意図しない接着を回避し，さらに展開が容易な方法を提案する．メッシュのマイクログリップ面にポリエチレン製シートを敷いて，ロール状にして中央を2-0絹糸で結ぶ．腹腔内に挿入して腹膜外側へ移動し内側上縁にメッシュを接着させる．糸を抜去しメッシュを少しずつ展開する．ポリエチレン製シートを少しずつ後面から引き抜き，ポリエチレン製シートがなくなった部分を接着させて展開は完了する．
In transabdominal preperitoneal approach（TAPP）, to the best of our knowledge, the procedure of mesh deployment has not been established. ProGripTM Laparoscopic Self-Fixating Mesh is fixed to the organs without traditional tack fixation, but sometimes the mesh is fixed to unintended tissues, such as the intestinal tract, the omentum, the peritoneum and etc. Our simple deployment method make it possible to avoid unintended adhesion. Polyethylene sheets was unfolded to the grip surface of the mesh and to roll the mesh. Also, the center is tied with the 2-0 silk thread. In the abdominal cavity, a polyethylene sheet was moved in the mesh and the mesh was moved outside the peritoneum, whose edge was fixed from inside. Then, thread was removed to roll out the mesh and the polyethylene sheet was removed, indicating the finish of the whole mesh development process.
