◆要旨：直腸癌手術におけるdouble stapling technique（DST）は再建の標準術式として確立されているが，残存直腸が長い症例などにおいては難渋する症例をしばしば経験する．筆者らが直腸DST吻合において円滑なcircular staplerの挿入のために行っている手術手技であるAir Enema法について報告する．Air Enema法とは，あらかじめ潤滑用ゼリーと空気を直腸内に注入してからcircular staplerを挿入しDST吻合を行う方法である．残存直腸を空気により拡張させることで，より安全で円滑なcircular staplerの挿入を助長できる．当院における通常挿入群とAir Enema群のcircular staplerの挿入時間を検討した結果，有意差をもってAir Enema群で挿入時間が短かった．Air Enema法を行うことでcircular staplerの挿入が容易となり円滑な直腸への挿入が可能となった.

Double stapling technique（DST） is established as a standard reconstruction in rectal surgery, but there have been some difficulties when the residual rectum is long. We have developed a method for smooth insertion of circular stapler in DST anastomosis. The new so-called Air Enema method is inserting circular stapler after injecting lubrication jelly and air to residual rectum. By expanding the residual rectum by air, the circular stapler is inserted more safely. Insertion time was significantly shorter in Air Enema method group than in the usual method group. Air Enema method is useful in DST anastomosis.
