腹腔鏡下手術時におけるトロッカーからの新しいセプラフィルムⓇ投入法（クラッカー法）の考案

New technique to facilitate insertion of a sodium hyaluronate-carboxycellulose membrane （SeprafilmⓇ） for laparoscopic surgery

◆要旨：癒着防止フィルム（セプラフィルム^Ⓡ）は腹部手術における腹腔内の術後癒着防止に有用であり，開腹手術時には多くの施設で使用されている．近年，手術手技の向上および定型化に伴い，泌尿器科・婦人科領域のみならず消化器外科領域でも腹腔鏡下手術症例が増加している．しかし，薄いフィルムであるセプラフィルム^Ⓡをトロッカーから腹腔内に投入する際の手技的困難さもあり，腹腔鏡下手術時にセプラフィルム^Ⓡを使用する施設は少ない．筆者らは円筒状のデバイスを用いて，セプラフィルム^Ⓡを簡便に腹腔内に投入できる手技（クラッカー法）を考案した．投入手技の詳細と臨床成績を提示する．

Sodium hyaluronate-carboxycellulose membrane（Seprafilm^Ⓡ） is useful for the prevention of peritoneal adhesion. Many institutions use Sepurafilm^Ⓡ for open surgery, but hesitate to use it for laparoscopic surgery because of the difficulty to insert it into the peritoneal cavity. We created a new technique to facilitate insertion of Seprafilm^Ⓡ from the trocar into the intraperitoneal cavity during laparoscopic surgery. This new method requires only the silk thread and the cylindrical tube. Sprafilm^Ⓡ could be inserted conveniently and with certainty from the trocar by this new technique compared with the previous reports. We report the procedure and results of this new method.
