当院における絞扼性イレウスに対する腹腔鏡手術の検討

Laparoscopic surgery for strangulating bowel obstruction : Analysis of surgical outcome in our hospital

◆要旨：［目的］近年，腹腔鏡手術の進歩により，絞扼性イレウスに対する腹腔鏡手術の報告が少数なされている．当院にて絞扼性イレウスに対し腹腔鏡手術を施行した症例について検討した．［対象］2009年1月〜2014年12月に絞扼性イレウスに対し腹腔鏡手術を施行した14例を対象とし，同時期に開腹手術を選択した14例と比較検討した．［結果］手術時間は両群間で有意差はみられなかったが，出血量および排便までの期間，術後在院日数は腹腔鏡手術群で統計学的に有意に良好な結果であった．［結語］絞扼性イレウスに対する腹腔鏡手術は，患者の全身状態が良好で，working spaceが確保でき，腹腔鏡下に安全に腸管を手繰る技術を有していれば，有用な術式と考える．

［Purpose］ Because of recent advances in laparoscopic procedures, cases of laparoscopic surgery have been reported for strangulating bowel obstruction. We examined laparoscopic surgery for strangulating bowel obstruction in our hospital. ［Material and Method］ During January 2009 and December 2009, 14 patients were diagnosed as having strangulating bowel obstruction and they underwent laparoscopic surgery. Results of these patients were compared with those of open surgery. ［Result］ There was no significant difference in operative time between the two groups. Blood loss was significantly lower, postoperative days of defecation and hospital stays were significantly shorter in the laparoscopic group. ［Conclusions］ Laparoscopic surgery for strangulating bowel obstruction may be considered as a useful method.
