急性呼吸窮迫症候群（ARDS）は1960年代に認識されて以来，高い死亡率を有しているが，よりよい支持療法や低1回換気量をはじめとした肺保護換気戦略などの確立により，多くの研究で経年的に短期予後の改善が得られたことが報告されている。生存率の改善とともに，退院後の長期予後に注目が集まりつつある。長期予後には生存率だけでなく，身体的，精神的，社会的障害における予後も含まれ，それぞれの要素に影響し得る急性期の管理についても関心が寄せられている。

　そこで本稿では，ARDS survivorsの長期予後における知見，ならびに影響を及ぼし得る因子や管理についてレビューする。

Acute respiratory distress syndrome (ARDS) has had a high mortality rate since it was first recognized in the 1960s. With the establishment of better supportive care and lung protective ventilation such as low tidal ventilation, many studies have reported improved short-term outcomes over time. Along with improvement in survival, ARDS is attracting attention for its long-term prognosis after discharge. Long-term prognosis includes not only survival but also the prognosis of associated physical, mental, and social disorders. There is also interest in managing the acute phase, which can affect associated disorders. In this article, we review the long-term prognosis of ARDS survivors, the factors and management that may influence it.
