The role of clinical engineers in planning for the use of medical devices and ICU lifelines in a disaster response

ICUにおける生命維持管理装置の使用は，電気，医療ガス（酸素，圧縮空気，吸引），水道などのライフラインにより支えられている。災害はその種類により求められる対応が異なるが，ライフラインの確保に対する事前計画を策定して備える必要がある。さらに災害時の新設部門の設置や病床の拡大などに際してもライフラインを考慮する必要がある。この事前計画において，日本のICUはどのような基準で設計されており，どこに限界があるのかを知る必要がある。また，策定された事前計画は，それに沿った訓練による習熟が必要となる。臨床工学技士は自施設の医療機器やICUライフラインに対する事前計画の立案に中心的にかかわり，事前計画に習熟した人員を育成するための訓練に積極的に参画するべきである。

The use of life support devices in the ICU is supported by ICU lifelines such as electricity, medical gas （oxygen, compressed air, suction） and water. The type of disaster response varies depending on the type of disaster. However, it is necessary to prepare a business continuity plan and an emergency response plan in advance to establish new departments, expand the number of beds, and maintain ICU lifelines. In this advance planning, we need to know what standards and limitations exist for ICUs in Japan. Proficiency in dealing with a disaster through training according to a plan is essential. Clinical engineers should be involved at every level in planning for the use of medical devices and ICU lifelines in their facilities, and should be actively involved in the training of personnel who are proficient in planning.
