Sparse evidence exists to guide when and how blood cultures should be obtained after the appropriate treatment of patients with bacteremia. In the case of Gram positive bacteremia, two sets of blood cultures should be obtained every 48 hours until the bacteremia is resolved with the hypothesis of infective endocarditis. As for Gram negative bacteria without successful source control, two sets of blood cultures should be obtained every 48 hours as well. Successful source control might render a follow-up blood culture unnecessary. The duration of therapy might be counted using the day that blood cultures become negative as the first day.
