精神科看護者の境界性人格障害に対するとらえ方と態度

Perspective and Attitude of Psychiatric Nurses toward the Borderline Personality Disorder

　境界例は，その病理から問題とされる行動も多く，治療，君護が難しいとされる疾患の1つである。この研究の目的は，精神科着護者が，看護が難しいと言われている境界例の患者に対しどのような経験や見解を持っているかを明らかにすることである。7病院の精神科に勤務する有資格の看護者に質問紙を配布したところ278名の回答が得られた。得られた回答を統計学パッケージHALBAUを用いて分析した結果，（1）看護者は境界例患者に対し共感的に理解しようとすれば自責的に自尊感情を低下させていくという関係がみられる。患者に対して否定的な感情を持つと，自己の力に対してあきらめ的な感情を持ち，回避的な対処をとる傾向がみられた。（2）境界例患者に対して肯定的な感情をもち，看護にやりがいを感じている看護者の特徴は，積極的対処をとることのほかにも，経験年数が長く，病気を器質的なものとしてとらえ，生活モデルを重視した看護を展開している，の2点が明らかになった。

　The purpose of this study was to clarify the perspective and attitude of psychiatric nurses toward borderline personality disorder patients.

　Research design was descriptive and exploratory. The questionnaire included the following are as ;

　1) the type and level of difficulties dealing with typical behavior of borderline personality patients, 2) the feeling of nurses toward patients as well as themselves, 3) the way of coping with behavior and willingness when nurses faced the above difficulties, 4) the perspective related to the etiology of borderline personality disorder, 5) the type of care model used to provide care.

　The subjects were collected from 278 psychiatric nurses from seven hospitals.

　The results were ;

　1) The nurses had a positive feeling toward patients but a negative feeling toward themselves.

　2) The nurses who had more empathy toward patients tended to feel more responsibility but overwhelmed by them.

　3) The nureses who had a negative feeling toward themselves tended to utilize avoidance coping with behavior.

　4 )The nurses who were willing to take care of patients tended to apply the care model which value daily living life and tended to believe the biomedical etiology.
