昨年新たに上梓された「脳卒中治療ガイドライン2009」の中で，初めて取り上げられた“その他の脳血管障害”に含まれる疾患の病態に基づく検査と診断について概説した．その多くは，頭部CT，MRIや神経超音波検査の進歩により，診断精度が向上し疾患概念そのものが明確になったものが多い．そのような観点から主な疾患については診断基準と代表的な画像所見を提示して診断に至る経緯を簡潔に述べた．ここで取り上げた疾患は一般の脳卒中と異なり症例数が比較的少ないため，エビデンスに基づく治療指針を提示することは困難であり，実地臨床における治療に際しては適切な検査に基づく個々の疾患の病態を把握することが特に重要と考えられる．
"Guidelines for stroke treatment in Japan, 2009" include several new chapters such as:"Other types of cerebrovascular disorders." The chapter reports cerebral arterial dissection, moyamoya disease, paradoxical cerebral embolism, cerebral venous sinus thrombosis, cerebral amyloid angiopathy, fibromuscular dysplasia, aortitis syndrome, hypertensive encephalopathy and vascular dementia. The diagnosis of these uncommon disorders has been able to be made precisely owing to progress in neuroimaging techniques including brain CT, MRI and neurosonology. For proper treatment of stroke due to these diseases, it is important to understand the pathophysiology underlying the disorder by carrying out appropriate examinations.
