　2004年に本邦で初めて発刊され，2009年11月に改定された脳卒中治療ガイドラインの作成過程，作成のコンセプト，推奨のグレードの理解の方法と読み方を概説した．さらにガイドラインを適切に遂行して，脳卒中の発症を予防し急性期治療を行いまた再発を予防するために，どのような臨床検査を，どのような考えで，どのように行うことが重要なのかを中心に，ガイドラインの内容を紹介した．

The process of preparation, the underlying concepts and the grading criteria of recommendations of the "Japanese Guidelines for the Management of Stroke 2004" and the revised 2009 Guidelines are described.　In addition, the clinical examinations and tests which are necessary for proper management of stroke patients are included, focusing on what tests are needed for primary and secondary stroke prevention, how they should be done, and when they should be done.　The role of t-PA treatment in the very acute stage of stroke is also discussed together with the necessary laboratory examinations.
