頭頸部末期がん症例における悪臭・漏出液処置の一工夫

A Newly Devised "Head and Neck Bag"

　I．はじめに

　末期がん患者は，全身的および局所的に種々の管理が要求されることは周知の事である。この管理の目的は，生存期間を長くしようとするものと，直接治療ではないが患者のcare（看護・世話）を向上させようとするものとに分けることができる。このcareの中で，特に頭頸部領域において，長期にわたってがん病巣部から生ずる強烈な悪臭・漏出液の処置は，日常臨床で筆者らがその対策に苦慮するところである。今回筆者らは，この悪臭を含んだ漏出液処置の対策として，悪臭・漏出液処置用装具を新たに考案し，主に末期がん症例に使用した結果，有用であったので，この装具の内容と使用経験を報告する。

　Management of secretion with offensive odour from lesions of the head and neck cancers of the late stage is a big problem to be solved. "Head and neck bag" made of polyvinyl was newly devised for accumulating the secretion. The bag is 60 cm long and has a stopcock at the lower end and a fastner at the upper end. The capacity of the bag is about 1,000 ml. In addition, it has adhensive tape which adheares the bag to the skin around lesions and prevents the secretion from leaking.

　This was used with success in management of the secretion from the lesions in one patient with cancer of the maxillary sinus, one with buccal cancer, and two with laryngeal cancer. Thus, not only patients but also their family, nurses, and doctors were released from a troublesome problem of the offensive odour and leaking of secretion.
