I．緒言

　blow out fractureとは眼前部外力による眼窩内骨折で，大部分の症例は複視などの眼症状を主訴とするため眼科を初診することが多い。

　われわれは今回大きな眼窩内骨折を伴いながら全く眼症状を訴えず，受傷後1年半以上経て反復する頬部腫脹を主訴として耳鼻咽喉科を初診した症例を経験し，手術により治癒せしめたので報告する。またこの症例の診断および経過を説明するにあたってCTスキャンが非常に有用であった。blow out fractureの診断，治療，とくに手術適応の決定などのさいの効果的なX線検査やCTスキャンの有用性についても言及したい。

　A 29-year-old female sustained a crushing injury to the left side of the face. At that time, she noticed no visual impairment and double vision. Two years later, she suffered from recurrent swelling on the left cheek. X-ray examination including coronal CT revealed a large fracture of the left orbital floor. Inferior rectus muscle and content of the orbit, however, remained intact. Antrotomy showed that the medial portion of the orbital floor fracture prevented drainage from the left antrum and caused recurrent cheek swelling. Coronal CT with variant window levels showed not only the fracture line but the extraocular muscles and ocular adipose tissue. So we recommend use of the coronal CT with variant window levels for the diagnosis of blow out fracture.
