Blow-in fracture of the orbit

　眼部にボールやげんこつなどにより外力が加わり，ちょうど眼窩内容が眼窩外へ圧排され眼窩底などに吹き抜け状の骨折が起こる　blow—out fractureはよく知られている。しかし外傷の機転によっては眼窩壁や眼窩縁が眼窩内へ骨折し，眼窩腔が縮小し眼球突出などを引き起こす，blow—in fractureも起こり得る。Blow-in fractureとは殴打などにより，眼窩部への強力な直達外力により，眼窩壁や眼窩縁が骨折し眼窩内へ転位し，眼窩腔が縮小して眼球突出や眼球の位置異常，運動障害，複視などを引き起こす骨折と考えられている。交通事故など重症外傷が原因となることが多い。筆者らは交通事故4例，飛び下り自殺1例の5例のblow-in fractureを経験した。骨折部位は眼窩外壁と内上縁骨折1例，外壁骨折1例，外壁と後壁骨折1例，上縁骨折2例であった。そのうち2例は骨折を眼窩内から除去または整復し複視も回復し経過良好であり，眼球破裂の1例は骨折を整復し義眼を装着した。後壁の視神経近くの骨折の1例は副腎皮質ステロイドによる保存療法で視力は回復した。上縁骨折の1例は本人は手術を希望せず，経過観察で複視はかなり回復した。

We report 5 cases of blow-in fracture of theorbit. The series comprised 4 males and 1 female.The ages ranged from 16 to 23 years. The fractureinvolved the lateral orbital wall in 2 cases, lateralwall and the medial roof in 1, orbital roof in 1 andmedial wall and the roof in 1. Traffic accident wasthe cause of orbital lesion in 4 cases.

We observed, as common features of blow-infracture, decrease in orbital volume resulting inproptosis, restricted ocular motility with diplopia.Surgical correction of the fractured orbital wallresulted in fair visual acuity and motility in 4 cases.Enucleation of the ruptured eyeglobe had to beperformed in one case.
