幼稚園耳科集団健診の検討—検査種目と施行時期

Screening of Ear Diseases in Kindergarten Children

I．はじめに

　子どもの聴力障害をできるだけ早く発見し，早く適切な措置を講ずる重要性については，わが国では近年になってようやく広く関心がもたれるようになった。また学童，生徒には学校保健法で選別聴力検査（以下聴検と略す），検診医の視診などの健診が義務づけられ，全国一応定期的に実施されているとみてよい。ところが学校保健法に基づく学校健診は幼稚園児にも準用されているが，園児にはこれらの健診が十分行われていないように思う。年少児では保健調査（アンケート調査）は親の主観に任せざるをえないし，聴検は信頼性，再現性が低い^6,7）うえに施行不能例が多い^{10,12,13,16,22）}し，鼓膜視診は検診医の個人差が大きく，集団健診では医師の労力がはなはだ大きいなどのためであろうか，3〜5歳児の耳科健診に関する報告は国内ではきわめて少ない^5）。園児への耳科健診は，無自覚性難聴（たとえば滲出性中耳炎，一側ろう，軽，中度の感音難聴など）の早期発見に役立つばかりか，難聴原因の究明，治療の早期実施が可能であり，さらに進路決定にも貴重な資料を提供する。

　ところで近年滲出性中耳炎（以後，滲中と略す）が幼児に急増している。柴原ら^4）は小学1年の難聴児の51％が滲中であったといい，Roberts^22）は要治療の中耳炎例は就学前幼児や小学低学年児童に最も多く，その37％は4歳以下であったと報告している。滲中の多くは20〜30dBの軽度難聴であるが，聴力が正常範囲にあるものも少なくない^3,9,14）。現行の聴検の選別レベルはオージオメータ出力目盛20dBであるので，滲中を正常耳として判断（under referral）する可能性が高くなる。また滲中の視診にはpneumatic otoscopeを用いないと適正な診断は難しい。集団健診では短時間内に多数の被検者を診察することを要求されるので，pneumatic otoscopeを使って視診する余裕はない。

　To determine the incidence of ear diseases, otoscopic examination, questionnaires regarding parent's opinion on their child's hearing ability and history of recurrent middle ear infections, tympanometry, screening audiometry at 20 dB and 30 dB, were performed in 180 kindergarten children aged 4 to 5 years.

　The study showed that these four methods had some difficulty in making a diagnosis of those ears without abnormalities (specificity) but greater difficulty in identifying those ears with abnormalities (sensitivity). However, tympanometry was found to be more accurate and valuable as an adjunct to pure-tone audiometry.

　The false positive for these methods was significantly increased in the winter months from February to the beginning of March, as compared with the summer from June to July.
