喉頭アミロイドーシスの2症例

Primary Amyloidosis of the Larynx: Report of Two Cases

I．はじめに

　アミロイドと呼ばれる線維性蛋白が組織間隙に沈着する疾患であるアミロイドーシスは，組織学的にある種の変性疾患と考えられているが，その発症病理は不明であり，従って診断や治療の面でも問題となる点が多く，厚生省特定疾患の一つとなっている。

　喉頭アミロイドーシスは，いわゆる喉頭良性腫瘍の1％にも満たない稀な疾患であり，治療としては切除手術がよく行われている。

　Two cases with laryngeal amyloidosis in 58 and 56-year-old women with a complaint of persistent hoarseness were reported.

　Laryngeal microsurgery showed the deposit of amyloid tissue at the right false vocal cord and the undersurface of bilateral vocal cords in the 1st case and at the left false vocal cord in the 2nd case. Results of physical and laboratory examinations were normal in both cases.

　Histological studies revealed that eosinophilic hyaline materials observed in H.E. stain were imparted the green twinkling in Congo red stain with polarization, and amyloid tissue in both cases did not exhibit any sensitivity to potassium permanganate.

　The endolaryngeal CO₂ laser microsurgery was performed for the 2nd case with good result.
