急性咽頭炎から食道周囲膿瘍と縦隔洞膿瘍を起こした糖尿病の1例

A Case of Primary Deep Neck Abscess and Mediastinal Abscess

I．はじめに

　頸部筋膜間隙に広範囲にひろがった炎症をdeepneck infectionといい，その原因は咽頭，喉頭，食道の損傷に二次的に感染した続発性のものと，単なる炎症から発展する原発性のものがあり，抗生物質の出現以後，後者は比較的稀となっている。

　最近，成人糖尿病患者で急性咽頭炎から咽後膿瘍，副咽膿瘍，皮下腫瘍，食道周囲膿瘍などのdeep neck abscessを起こし，さらに縦隔洞膿瘍へと伸展したが，幸いに治癒しえた1例を経験したので報告する。さらにその成因について若干の考察を行った。

　A 46-year-old female with diabetes mellitus for two years developed abscesses of the deep neck and mediastinum.

　The causes of mediastinal abscess extended from the deep neck infection were considered as follows : 1) Uncontrolled diabetes mellitus.

　2) The causative microorganisms of abscess formation was considered to be gasforming anaerobic bacteria.

　3) The timing of the surgical drainage of the periesophageal abscess was delayed.

　The patient was cured with antimicrobial therapy in a large dosis and extensive surgical drainage combined with controlling diabetes mellitus.
