　I．はじめに

　中耳と内耳は前庭窓，蝸牛窓で接している。前庭窓にはあぶみ骨が存在するために，耳硬化症をはじめとして，この部分は手術的に操作されることが多く，前庭窓，あぶみ骨への到達法は完成しているといってよいであろう。これに反して，蝸牛窓は解剖的にみえにくい部位にあり，しかも疾患が少なく，奇形，あるいは耳硬化症による蝸牛窓の閉鎖症例に対して稀に手術の対象となるにすぎなかった。しかし，最近になって蝸牛窓膜破裂症の存在がみとめられ，その診断治療に蝸牛窓の手術が行われるようになってきた。また，良性発作性頭位眩量症に対して行われるsingular neurectomyに際しては，蝸牛窓膜を明視下におく必要があり，蝸牛窓に対する手術が今後より一般化されることになるものと考える。

　本稿では蝸牛窓に対する手術的到達法について述べてみたい。

　A surgical approach to the round window and its neighbouring structures was described with illustrations. The round window area was exposed after reflecting a wide tympanomeatal flap. Bony annulus and rim of the round window were curetted. This procedure was insufficient to observe the round window membrane. After an additional postauricular sulcus incision, bony wall of the external meatus was widely exposed. We were able to observe the whole circumference of the round window membrane by lowering the inferior wall of the external meatus.
