　I．緒言

　慢性中耳炎とくに中耳根本手術後の症例には，耳漏が持続し，聴力は聾またはそれに近い高度難聴となり，さらにめまいや瘻孔症状を伴っている症例がある。このような症例は難治であることが多く，中耳根本手術またはその拡大術式によっても，完全に上皮化，乾燥し後治療を必要としない中耳・乳突腔とすることが困難なことが稀ではない。再三の手術によっても耳漏やめまいが持続し，止むなく長期間にわたる後治療を続けている症例すら経験される。我々はこのような13症例に乳突・中耳・外耳道の全閉塞術を行い，満足すべき成績をおさめたので，ここに手術方法，手術成績とくに長期追跡結果を報告し，本術式の適応について考察を行う。

　Primary closure of the external auditory meatus following total obliteration of the mastoid, tympanic cavity and external auditory canal was employed in 13 patients with unilateral severely damaged ear having uncontrollable chronic otorrhea. Eleven underwent one or more previous unsuccessful surgeries, and 9 had vertigo. All ears but one with radionecrosis of the temporal bone healed by three weeks postoperatively. Indications of this procedure were discussed. For safe and successful obliteration emphasis was placed on meticulous removal of diseased cells, granulation tissue, cholesteatoma, and canal skin.
