　I．はじめに

　聴性脳幹反応（auditory brainstem response：以下ABRと略す）はJewett^1,2），sohmerら^3,4）によって初めて報告されて以来，聴覚系のみならず，脳神経学的にも検討がつみ重ねられ，その臨床応用範囲が広げられてきている。このうち中枢神経系疾患のABRについても数多くの報告がみられるようになってきた。

　本邦でもすでに脳幹部に病変を持った患者のABRがいくつか報告されてきている（大西^12），細川^13,14），加我^15），神崎^16），柏木^17））。近年は脳幹部病変の中でも蝸牛神経核付近の病巣，特にいわゆる小脳橋角部腫瘍に対する早期診断法としてABRの有用性が論じられてきている。

　Auditory brainstem responses (ABR) was discussed in a case of asymptomatic acoustic neurinoma in a 20-year-old otherwise healthy woman. ABR records revealed abnormal wave pattern, and wave 3 to 5 were markedly prolonged in latency, and the lower brainstem lesion was suggested. Neuroradiological examinations revealed a mass at the right cerebellopontine angle, but other conventional examinations were all negative. After the removal of the tumor, she returned to work without difficulty except a total hearing loss of her right ear.
