　I．はじめに

　頸部腫瘤は，日常よく遭遇する疾患であるが，その良・悪性，原発巣か転移巣かの臨床診断は必ずしも容易でない。頸部リンパ節へ転移する原発巣としては，その解剖学的関係より頭頸部領域および胸腹部臓器が考えられる。頸部リンパ節転移癌の原発巣を種々の検索にもかかわらず見出し得ず，転移巣にのみ治療を行わざるを得ない症例に遭遇することも稀ではない。また，原発巣発見までの時間的経過によっては，予後を左右する可能性がある。それゆえ，的確に早く原発巣を診断するためには，生検材料の組織学的検索と臨床データとを併せ検討する必要がある。

　筆者らは，昭和43年1月から昭和53年12月までの11年間に国立松山病院を訪れた患者のうち，頸部腫瘤を主訴または初発症状として来院し，生検または手術により臨床病理学的に検討し得た1053例について主として耳鼻咽喉科領域より転移性頸部腫瘤を解析した。

　We have investigated on 1053 patients with achief complaint or primary symptoms referring to cervical masses seen at Matsuyama National Hospital during the period of 11 years from January 1968 through December 1978.

　1) There were 303 cases of thyroid diseases and 750 cases of non-thyroid diseases. The latter included 282 cases of inflammation (37.6%), 430 cases of tumors (57. 4%) and 38 cases of congenital diseases and miscellaneous.

　2) Out of 430 cases of tumors, primary malignant tumors were found in 122 cases and metastasized tumors were in 172 cases including 51 cases of primary sites in the head and neck region.

　3) For the diagnosis of the cervical masses, it is important to assess the clinical findings with the pathological reports. When the masses are histologically proved as metastatic tumors, it is mandatory to investigate further the primary site.
