I．はじめに
耳下腺部の腫瘍には，日常臨床時にしばしば遭遇するが，術前に明確な病名診断を下すことが困難なことが多い。特に良性腫瘍の場合にこの傾向がある。これは触診や視診で耳下腺部の腫瘍の性質を知ることに限界があること，およびこの部は胎生学的にみて種々雑多な組織を基盤にもつ腫瘍が発生し得るからである。
私どもは術前に耳下腺部腫瘍と診断し，手術により環椎の横突起の過長により形づくられた偽腫瘍であることが判明した1例を経験したので報告する。
A 59 year old woman complained of a massin the right parotid region with occasional pain. Sialography revealed the presence of sizable mass in the deep lobe of the parotid gland.
A surgical exploration was performed under a tenetive diagnosis of pleomorphic adenoma of the parotid gland. However, the parotid gland was found to be intact. Under the posterior bellyof the digastric muscle, the transverse process of the atlas was found to be protruding laterally. The tip of this bone was sectioned for approximately 1 cm.
The patient has been free from symptoms for 2 years following surgery. No mass could be further palpated.
