CTによる過長茎状突起症候群の検討

Evaluation of Elongated Styloid Process by Computed Tomography

　緒言

　過長茎状突起症候群あるいはEagle症候群は咽頭粘膜に異常を認めないにもかかわらず咽頭異物感，嚥下痛，耳痛などを呈する疾患で，慢性の咽頭症状の患者の鑑別診断として忘れてはならない疾患のひとつである。

　この疾患は扁桃洞の触診や単純写真で確認されるのが従来の一般的方法である^1,2）。しかしながら，単純X線像においては解剖学的に下顎骨および歯の陰影と茎状突起は重なり易く，加えて，茎状舌骨靱帯骨化を不連続に伴うような症例^3〜6）では正確な判読が必ずしも容易ではない。また，過長茎状突起は必ずしも症状を伴わないことが知られており，単に長さのみならずその走行と周囲組織の位置関係が症状発現に関与していると考えられている^7）。

Elongated styloid process syndrome causes cra-niofacial and cervical pain especially a dull pharyn-geal pain as seen in chronic pharyngitis. In the present study, the styloid process in this syndrome was examined by CT scan. As a result, this pro-cess was significantly longer and its tip was signifi-cantly closer to the pharyngeal wall in the patients with this syndrome than in the normal controls. Although the elongatd styloid process is not always symptomatic, these findings seem to be closely related to the occurrence of the symptoms. Further studies along this line will shed light on the signi-ficance of these morphological findings.
