外傷性顔面神経麻痺に対する顔面神経減荷術

Facial Nerve Decompression of Facial Palsy Following Head Injury

　はじめに

　近年，交通事故の増加に伴い，外傷性顔面神経麻痺の患者は増加している。これらの外傷性顔面神経麻痺の治療としては，保存的治療と手術療法とがある。手術療法としては，顔面神経減荷術（以下，減荷術と略）を行うことが多いが，その適応や施行時期については議論のあるところである。私達は最近11年間に42例の外傷性顔面神経麻痺を経験し，このうち27例に対し減荷術を施行した。今回この27例を検討し，若干の知見が得られたので報告する。

Twenty-seven patients with facial palsy resulting from head injury were treated with facial nerve decompression between 1989 and 1991.

We performed a transmastoid approach that ex-posed from geniculate ganglion to the stylomastoid foramen.

Facial palsy was improved in 95%, including 52% who showed complete recovery.
