　Ⅰ．緒言

　食道鏡は気管支鏡とともに異物摘出の目的で使用され出して以来，最近では疾患の診断あるいは治療の目的で使用される機会が多くなり，また食道鏡にも幾多の改良が加えられその操作方法も進歩したにもかかわらず，最近の使用頻度からみて食道鏡検査による偶発症，合併症が多くなつており，しかもそれらは重篤なものであることが少くない。

　我々は吐血を主訴とする症例に診断の目的で食道鏡検査を実施し，その直後高度の呼吸困難を伴う気胸を合併したが，酸素吸入と抗生物質の投与並びに開胸排気によつて軽快した一症例を経験したのでその大要を報告する。

Accidental pneumothorax was caused duri-ng the diagnostic procedure of esophagusco-py in a patient who complained of hemopto-sis. The patient was cured by following the regime of absolute bed rest without any food through the esophagus, ample administration of antibiotics and oxygen and of open remo-val of air from the thorax. The authors assumed the cause of the pneumothorax was probably instigated by an increase of lung air pressure created during esophagoscopy on this senile patient and under which the lung air cell ruptured.
