メニエル氏病に於けるヒスタミン皮内反応の再検討

　I．緒言

　Atkinson（1941）はメニエル氏病（以下「メ氏病」と略記）患者に於いてヒスミン皮内反応を試み，その成績によつて此を2群に分類した。1群はヒスタミン皮内反応（以下「ヒ反応」と略記）が陽性のもので，アレルギー性に由来し，発作の機転は発作性に来たる血管の拡張であつて，此の場合ヒスタミン脱感作が有効である。又院残りの1群は「ヒ反応」が陰性で，その発作の機序は血管の収縮によるものであつて，この場合ニコチン酸が有効である。」と，斯るAtkinsonの説には2つの重要な臨床上の問題が含まれている。1つはアレルギーとの関連に於いて，又もう1つは末梢血管（特に内耳血管）の機能上の問題と関連して，「ヒ」反応の果す役割である。

　1861年Ménièreは内耳障害によつて眩暈が起り得る事を報告したが，その診断，治療に当つては，単に迷路に於ける局所的原因のみならず，発作の背景となつた全身的の条件を広く考慮する必要がある。その中で，若し発作の成立機転がアレルギーと関連性があると想像される場合に此を他覚的に簡単に検出する力法があれば極めて便利である。又同様に若し発作の成立機転が末梢血管機能と関連があると考えられる場合，此を他覚的に検出する方法があれば幸である。

　Re-evaluation of histamin intradermal test for Meniere's disease is conducted with 40 cases as material of study. Pseudopodia 25 mm or over in their diamter were considered as a positive histamin reaction and those that developed pseudopods of extended swellings counting more than 3 in number within the test period of 5, 10 and 15 minutes were considered as allergic reactions.

　Among 19 cases of Meniere's diease 5 cases showed a positive reaction with the above test. Since sympathicolysis was established in all cases such reactions were considered as those derived in connection with abnormal vascular functional reaction rather than allergic.
