I．はじめに

　蝶形骨洞の発達には個人差があり，気胞化が亢じると，翼突板基部から更には翼状突起内部にまで進展することがある^{2,4,9-12,14）}．この蝶形骨洞の翼状突起内部への進展部はpterygoid（or inferolateral）extension of the sphenoid sinusと呼ばれる^10）．

　今回われわれは中頭蓋窩原発の髄膜腫が側頭下窩及び上記のpterygoid extension of the sphenoid sinusへ進展していた症例を経験した．中頭蓋窩から側頭下窩に進展した腫瘍性病変に対しては，側頭下窩を大きく開放する種々のアプローチが考案されている^{1,6-9,13,15-18）}．通常であれば腫瘍が翼突板基部〜翼状突起へ浸潤している場合，三叉神経第2枝，翼口蓋神経節，翼突管神経（Vidian nerve）等の障害を恐れ，なかなか機能温存を図りながらの腫瘍全摘出に踏み切れないが，このような空隙が存在し，翼状突起の範囲が内面より境界されている症例では，この空隙を利用して腫瘍摘出を試みる余地が生まれる．症例を呈示し，pterygoid extension of the sphenoid sinusへの外科的アプローチ法について検討した．

A case of middle cranial fossa meningioma extending into infratemporal fossa and the pterygoid process is presented. The patient had a sphenoid sinus extending inferolaterally into the pterygoid process, which is known as pterygoid extension of the sphenoid sinus. This type of variation of the sphenoid sinus allowed a safe and well-oriented approach to the pterygoid pro-cess via the infratemporal fossa. The tumor extending into the pterygoid process was removed successfully without damaging any surrounding structures, e.g. max-illary nerve or Vidian nerve. However, pterygoid exten-sion of the sinus is seen in only 40% of cases. There-fore, close preoperative examination with bone window CT scan is mandatory before employing this approach.
