I．はじめに

　三叉神経鞘腫は，CTが普及した今日でも，術前に他の中頭蓋窩腫瘍と鑑別することは難しい．一方，MRIの導入により，詳細な解剖学的情報を得ることができるようになった．今回，われわれは腫瘍内出血で発症し，臨床経過および画像所見等，興味ある所見を呈し，手術的に摘出した海綿静脈洞部三叉神経鞘腫の1例を経験したので若干の文献的考察を加えて報告する．

A case of trigeminal neurinoma located in the left cavernous sinus and revealed by intratumoral hemor-rhage is reported. Preoperatively, magnetic resonance imaging provided the accurate localization of the lesion, allowing precise planning of the surgical approach. The neurinoma inside the cavernous sinus wall was re-moved successfully, and the patient developed no new neurological deficits except for anesthesia in the 1st di-vision of the trigeminal nerve. The characteristic find-ings of magnetic resonance imaging of the trigeminal neurinoma in the cavernous sinus are reported.
