I．はじめに

　大脳皮質下出血は脳動静脈奇形，脳腫瘍，出血性素因などの基礎疾患を有する場合発生しやすいことが知られている．しかし実際にはこのような基礎疾患を確認できる症例はむしろ少なく，出血原因を特定できない特発性大脳皮質下出血が多い．今回われわれは大脳皮質下出血で発症した症例に対し脳血管撮影を施行したところ，出血側の脳主幹動脈閉塞とよく発達したleptomeningealanastomosisが認められた症例を2例経験したので，その皮質下出血の原因が脆弱な側副血行路にあるのではないかと考え，若干の考察を加え報告する．

Abstract

We report two cases of asymptomatic occlusion of the main trunk of the cerebral artery associated with subcortical hemorrhage in the area fed by collateral cir-culation.

The first patient, a 51-year-old female who had suf-fered from untreated hypertension for 20 years, was hospitalized in a state of coma. Computed tomography (CT) revealed a subcortical hemorrhage in the right parietal lobe as well as subdural and subarachnoid hemorrhages. Cerebral angiography disclosed occlusion of the right internal carotid artery at its origin.
